The following tools are intended to help you learn your migraine triggers, determine their impact on your life – and the lives of those around you – as well as providing valuable insight for your health care providers as they help you find the best course of treatment for your migraines. The more high-quality information you can provide to your doctor, the easier and more accurate your diagnosis will be. Spending a little time with these tools, and sharing the results with your doctor, can help you find the best way to manage your migraines and minimize their impact.

Doctor Discussion Guide

The Doctor Discussion Guide helps you provide very specific input into your next doctor’s visit. This online quiz helps you identify the following:

  • the location and intensity of your migraine pain
  • the additional symptoms you experience
  • the time of day and number of headaches you typically
  • the impact your migraine days have on work, social
    relationships, and more

By completing (and printing!) the online report, you’ll be giving your health care useful information that can help them make an appropriate diagnosis and design a treatment program that works for you.

Headache Impact Test (HIT-6TM) Visit Site

The Headache Impact Test, or HIT-6, is a six-question report that results in a score that can be used to provide insight into the impact your headaches are having on your life. The questionnaire will help you and your doctor understand the following:

  • the level of pain you endure when you have a headache
  • the level of stress your headaches cause
  • the impact your headaches have on your ability to function

Migraine Disability Assessment (MIDAS©)

The Migraine Disability Assessment is a tool that helps migraine sufferers measure the impact their migraines have on their lives, including work and family responsibilities. The tool assesses your migraine disability in the following areas:

  • impact to your work and school schedule
  • impact on your household responsibilities
  • impact on your ability to attend family and social activities

The questionnaire will help your primary care physician understand your individual level of pain and the impact your migraines have on your ability to be a productive and contributing member of society. Migraine Trigger Chart

The Migraine Trigger Chart is a handy guide to quickly identify your common triggers and provide that information to your health care provider. Of course, it’s important to remember that not all migraines are started by triggers – some migraines simply occur on their own. The chart helps you determine environmental and food triggers. It also helps you determine physical or emotional changes that may be triggering your migraines as well.
